急性肾损伤(acute kidney injury,AKI)是由多种原因导致的临床综合征,主要表现为肾组织损伤、肾小球滤过率急骤下降,导致肾脏对溶质排泄和水、电解质调节障碍以及全身各系统症状[1]。流行病学显示,高收入国家中,50%的AKI发生在医院,主要诱因为脓毒症、肾毒性药物及侵入性操作;中低收入国家中,77%的AKI为社区获得,主要诱因为感染及低血容量性休克[2]。约10%的AKI患者需要进行肾脏替代治疗(kidney replacement therapy,KRT),尽管KRT已被广泛应用,在ICU患者中AKI的病死率仍高达50%[3],即使在非ICU住院的AKI患者中也高达10%~20%[4]。近些年有关AKI的基础及临床研究均取得一定进展,但因其发病及进展机制复杂、影响因素较多,至今尚未完全阐明。
