老年重症新型冠状病毒感染住院患者营养评估的研究进展

新型冠状病毒感染(corona virus disease 2019,COVID-19)大流行为全世界的患者和医疗系统带来了挑战和威胁,该病主要累及呼吸道,部分患者在发病一周后进展为重症病毒性肺炎,并可能恶化为多器官功能衰竭并致命。尽管COVID-19影响所有人群,但大多数的住院及死亡病例发生在老年人中,受影响最严重的是70 ~ 80岁的老年人,以及同时患有肥胖、心血管疾病、高血压和糖尿病等合并症的老年衰弱患者[1],其预后更差,病死率更高。有研究表明,良好的营养状况可以降低发病风险并改善预后,营养缺乏则可能增加COVID-19患者发生严重感染的风险[2]。因此,在治疗过程中应充分考虑营养不良的评估,将老年重症COVID-19患者的营养评估纳入整体治疗策略。目前对于老年重症COVID-19患者的营养风险评估知之甚少,大多参照既往危重症患者营养支持治疗相关指南和文献的临床经验,应用于老年重症COVID-19患者尚需要一定的经验积累。本文对COVID-19流行期间老年重症COVID-19住院患者营养评估的研究进展做一综述。
